The Search for an Ideal Temporary Skin Substitute: AWBAT

چکیده

OBJECTIVE The search for an ideal temporary skin substitute is a continuous quest. Without the ability to provide active transport systems powered by adenosine triphosphate or adenosine diphosphate that pump fluid out on demand, all skin substitutes, however effective, would be a compromise. Therefore, the best that any current wound covering design can do is to strive to produce all the other qualities of an ideal skin substitute. Recently developed technology utilized in AW BAT attempts to better maximize those attributes. METHODS Desirable traits of an ideal skin substitute include adherence, moisture permeability control, infection control, safety, pain management, physical adaptability, transparency, stability, cost-effectiveness, and ease of application and removal. Analysis of the optimization of these traits exemplifies the proficiency of a skin substitute. RESULTS Improvements in porosity and manufacturing methodology intend to increase AWBAT's capability over existing products to better fulfill the ideal properties of a skin substitute. CONCLUSION It is expected that new technological improvements in AWBAT will provide improved performance over existing skin substitutes. Increased porosity and continuity of the 3-dimensional silicone-nylon membrane are expected to improve acute adherence and reduce the potential for infection associated with fluid accumulation, and the elimination of cross-linking agents in the collagen application is expected to improve the interaction with fibrin and eliminate the potential for allergic reaction to those agents.

The Search for an Ideal Temporary Skin Substitute: AWBAT Plus, a Combination Product Wound Dressing Medical Device

OBJECTIVE To create an ideal temporary skin substitute. METHODS Temporary skin substitute (Biobrane, AWBAT and AWBAT Plus) porosity, 3D matrix, and biochemical composition were evaluated for impact on healing wounds (pain, fluid accumulation, infection, and time to heal). Twenty-four Sprague-Dawley [H1a: (SD) CVF] rats were used to measure the histology of healed full-thickness wounds. Tissue...

AWBATTM: Early Clinical Experience

OBJECTIVE The purpose of this article is to describe the early clinical experience with AWBAT. METHODS Burn patients requiring (1) donor sites or (2) treatment of a superficial burn wound injury were treated. A total of 45 patients with 69 distinct wounds were included. AWBAT-D was evaluated in donor sites and AWBAT-S was evaluated in superficial partial-thickness burns. Days to healing, pain...

Phase I/II clinical evaluation of StrataGraft: a consistent, pathogen-free human skin substitute.

BACKGROUND Large wounds often require temporary allograft placement to optimize the wound bed and prevent infection until permanent closure is feasible. We developed and clinically tested a second-generation living human skin substitute (StrataGraft). StrataGraft provides both a dermis and a fully-stratified, biologically-functional epidermis generated from a pathogen-free, long-lived human ker...

Evolution of a Biosynthetic Temporary Skin Substitute: A Preliminary Study

OBJECTIVE To compare PermeaDerm to first temporary biosynthetic skin substitute (Biobrane, cleared by the Food and Drug Administration in 1979). METHODS Different temporary skin substitutes (Biobrane, PermeaDerm, and PermeaDerm derivatives) were tested for physical differences, impact on healing wounds, inflammatory response, and ability to allow adequate growth of dermal fibroblasts and mese...

Soluble Leishmanin as an Ideal Reagent for Skin Testing in Human Leishmaniasis

Delayed-type hypersensitivity reaction measured by skin testing, is an important tool for evaluation of cellular immune response in leishmaniasis and has been used recently as an indication of exposure to infection. Skin testing in leishmania infection needs a standard reagent with high specificity, sen‌sitivity, and potency. In the present work, a soluble leishmanin, and three whole parasite p...
